I understand upon withdrawal of lessons I must inform instructor by email by the 15th of the month prior to withdrawal or following month's tuition is due in full.(required)

I understand that I am signing up for a specific weekly time for lessons and that I must make every effort to keep that time weekly, to not schedule other appointments at that time and that re-scheduling is not guaranteed.(required)

I understand that notice, by email, must be given, by noon the day prior to missing a lesson, to be eligible for a rescheduled make up lesson.(required)

I understand, Once a lesson is re-scheduled there will be NO further changes.(required)

I understand, Tuition is due by last lesson of the month for following month(required)
